What is Massachusetts Form 1099-HC?
Massachusetts implemented the Individual health coverage mandates at the state level beginning on January 1, 2006. This mandate requires all Massachusetts residents over the age of 18 to acquire Minimum Creditable Coverage (MCC).
The state requires employers to report certain information about the health insurance coverage offered to their employees who reside in Massachusetts. Therefore, the state designed the MA Form 1099-HC to gather this information from employers.

For employers with fully insured plans, most insurance carriers will submit forms MA 1099-HC to report minimum creditable coverage to the state and distribute copies to the employees on behalf of the employers. If their insurance carrier fails to file, employers are responsible for fulfilling these obligations to avoid penalties.
Employers offering self-insured plans are responsible for submitting Form MA 1099-HC to report their minimum creditable coverage to the state and distrib ute copies of the form to their employees.

Information required to file MA Form
1099-HC?
To e-file MA Form 1099-HC, you must need the following information:
The name and FID (Federal Tax ID) of the responsible insurance company or administrator.
The name, subscriber number, Date of Birth (DOB), and address of the primary subscriber.
The coverage period during which the MCC was offered to the subscriber and their dependents.
If the subscriber was covered for all 12 months of the year, the box indicating this must be checked.
If the subscriber was not covered for all 12 months of the year, the box against the months the MCC was offered must be checked.

Employers must file MA Form 1099-HC electronically to the Department of Revenue.
Paper filing the Form MA 1099-HC with the Department of Revenue is not permitted.
